What is a Patient-Centered Medical Home?

Patient engagement is integral to high-quality health care and the Patient Centered Medical Home sometimes referred to as the Primary Care Medical Home or simply Medical Home.

A patient-centered medical home integrates patients as active participants in their own health and well-being. Patients are cared for by a physician who leads the medical team that coordinates all aspects of preventive, acute and chronic needs of patients using the best available evidence and appropriate technology. These relationships offer patients comfort, convenience, and optimal health throughout their lifetimes. - American Academy of Family Physicians, May 2008. 

Technology offers the promise of making patient engagement more natural and less burdensome for both families and practices using computer and Internet-based tools to support communication between families and providers. Also to provide information, support skills training, allow networking among families, facilitate connections between health care providers and between health and other social services, and enable patient feedback on care.
